Appointment Dates and Times Members must notify Fresh-Faced of a change in their scheduled appointment date or time 24 hours prior to the scheduled appointment. A $25 No-Show fee will be charged to the member if proper notification within the above time frame is not provided by the member. Additionally, if facial enhancements are not pre-booked, massage time may need to be reduced. The appointment time may need to be extended at the sole discretion of Fresh-Faced.
Late arrivals Members arriving 15 minutes or more late to an appointment will result in either the appointment being rescheduled or the facial time being reduced accordingly, at the sole discretion of Fresh-Faced.
Missed/Gifted Facials If a member misses a monthly facial, the member may make up the missed facials in the following months for each missed facial that is not expired and the esthetician approved the frequency of the skincare appointments. A member may also gift each unexpired missed facial. Gifted facials will carry an expiration date of 6 months after the facial is gifted. Gifted facials cannot later be used by the member for the member.
Expiration/Voiding of Facials Each monthly missed facial expires within six months from the date each monthly facial was missed. Prepaid Package purchases of six facials expire within one year of purchase. Unused expired missed monthly facials or unused expired package facials will not result in a refund. Cancellation or termination of this agreement voids all missed or unused facials up to the date of termination.
Cancellation by Member Member may cancel this agreement at any time for any reason by giving written notice, either in person or through email to freshfacedsa@yahoo.com 30 days prior to the next monthly billing cycle draft. Member is responsible for all monthly drafts incurred until cancellation becomes effective. Cancellation by member is effective on the first day of the month of the next billing cycle draft following the receipt by Fresh-Faced of the cancellation notice. Fresh-Faced is not obligated to refund and monies paid or give credit for any unused unexpired missed facials upon written notice of cancellation. Memberships cannot be suspended.
Termination by Fresh-Faced Fresh-Faced may terminate Monthly Memberships immediately after:
1. Three(3) consecutive declined monthly billing cycle drafts.
And/or
2. Three(3) consecutive months of no appointments.
And/or
3. Member fails to follow Fresh-Faced policies and procedures.
Package memberships may be terminated by Fresh-Faced if member fails to follow Fresh-Faced policies and procedures. Unused missed monthly facials or unused package facials will not result in a refund. Termination of this agreement voids all missed or unused facials up to the date of termination.
Policies and Procedures Member agrees to provide Fresh-Faced with current, accurate, complete, and updated information including Member’s name, physical address, email address, telephone number, and applicable payment data. Member also agrees to advise Fresh-Faced of any limitations or sensitivities that may affect treatment and to inform the esthetician if member feels any discomfort during any session. Fresh-Faced reserves the right to change its fees, policies or procedures at any time upon 30 days’ notice via email.
